By using our services, website, or scheduling an appointment, you agree to the terms outlined below. These Terms & Conditions are intended to protect both your rights and ours, while maintaining transparency in operations.

1.) Use of Services

ProCheck Services provides mobile and in-office specimen collection services for DNA testing, drug and alcohol testing, fingerprinting, and related services. By booking with us, you confirm that:

• You are at least 18 years of age or have parental/legal guardian consent.

• The information you provide is accurate and complete.

• You understand that ProCheck Services does not provide medical advice, diagnosis, or legal conclusions.

2.) Payment & Refund Policy

All services must be paid for in full prior to collection. Once testing has begun or kits have been shipped, all sales are final. Refunds are only available in accordance with our Refunds & Cancellations Policy.

Same-day cancellations or no-shows are subject to a $50 administrative fee.

3.) Test Results

Turnaround times provided at scheduling are estimates. While we work to ensure timely processing, delays may occasionally occur due to lab processing, specimen issues, or third-party circumstances.

Results are delivered via the method discussed during your appointment (e.g., secure email or direct call). We do not guarantee specific outcomes and are not liable for how results are interpreted or used by third parties.

4.) Confidentiality

We respect your privacy. All personal data and test results are handled in compliance with applicable privacy laws. We do not share your information without your explicit consent unless required by law or legal order.

5.) Identification Requirements

All individuals being tested must present valid government-issued photo identification at the time of collection. Failure to do so may result in cancellation of the appointment and associated administrative fees.

6.) Limitation of Liability

ProCheck Services is not liable for:

• Any damages resulting from the misuse or misinterpretation of test results.

• Delays outside of our control.

• Refusals or errors caused by third-party laboratories or agenices.

7.) Right to Refuse Service

We reserve the right to refuse service to anyone for any reason, including but not limited to failure to comply with identification requirements, disruptive behavior, or inappropriate conduct.

8.) Modification to Terms

ProCheck Services reserves the right to update these Terms & Conditions at any time. Continued use of our services after changes indicate acceptance of the updated terms.
